FLSA Status Non-ExemptJob Role Summary The Sandra Eskenazi Mental Health Center Employment Consultant provides assistance to individuals with physical and/or psychiatric disabilities in obtaining and retaining competitive work in a community setting. This position is responsible for orienting, training, coaching and supporting individuals to make placements successful.Essential Functions and Responsibilities Provides assistance to individuals with physical and/or psychiatric disabilities in obtaining and retaining competitive work in a community settingResponsible for orienting, training, coaching and supporting individuals to make placements successfulCompletes intake and assessment to determine optimal employment opportunity for each individual, including assessment for vocational interest, support systems, past/present problem areas, assets/potentials, skill and needsFormulates a vocational rehabilitation plan in collaboration with the individual and their primary Clinician/Care Coordinators and others as requested by the individual.Participates in interdisciplinary meetings and scheduled conferences providing appropriate inputCarries out job matching/development through comprehensive search in community for potential employment sources which includes making contacts with potential employers and developing ongoing working relationship with themDevelops job specifically to match patient needsCompletes all required client/service activity forms to ensure accurate information for client billing, setting fees and management information according to the Center policies and proceduresCompletes FSSA documentation and record keeping as requiredMeets the agency standards of timely completion of all documentationMeets the agency standards of service hoursJob Requirements Bachelor's degree required, with the ability to meet minimum competency standards set forth by Eskenazi Health Sandra Eskenazi Mental Health Center CMHValid Indiana Driver's LicenseKnowledge, Skills & Abilities Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data/interpret appropriate information needed to identify each patient's requirements relative to age specific needsKnowledge of the process and parameter of severe mental illness, problems and potentials of patients, be patient centered, committed to principles of support employmentRecognize people's inherent right to work and to make choices concerning their workEstablish contacts with regard to Sandra Eskenazi Mental Health Center /Community resources, information, forms that are neededFamiliar with business sector protocol and procedures, public relations, and marketing skills that can be applied to assist individuals with disabilitiesKnowledge of the process and philosophy of supportive employmentVerbal and written communication skills neededAbility to:Use age appropriate communication skillsUse variety of reality oriented problem solving counseling and assessment techniquesCommunicate verbally and in writing in a clear thorough/concise manner (writ¬ing must be legible)Organize and plan goals/objectives and prioritize such goalsWork cooperatively with diverse individuals consisting of severely mentally ill, mental health professionals, community business leaders/co-workers in work setting, family and significant others
