• Conduct comprehensive evaluations and develop patient-centered treatment plans
• Collaborate with physicians, case managers, and fellow rehab team members
• Communicate with patients and families to ensure understanding and continuity of care
• Support various appointment types and adapt care plans as needed Principal Accountabilities: Essential functions include patient evaluation, establishing plans of care and treatment programs, providing interventions, providing patient and family education, documenting interventions, and billing appropriately.
