  • Shift: DayProvides comprehensive nursing services to patients and their families in a community setting, with an emphasis on the promotion and maintenance of health, early detection, and restorative measures. While this care is provided under the leadership of an RN Case Manager or the Director of Nursing, staff nurses are some of the patients’ closest advocates. Completes Home Health documentation on patients and their families to ascertain the level of in-home care needs, including physical, emotional, and mental needs, including Start of Care, Recertifications, Discharge and Resumption of Care assessments. Assists the primary care physician in developing a plan of care and maintains appropriate communication with the physician throughout the course of treatment. Implements and manages a plan of care under the supervision of the Nursing Case Manager, Home Health Administrator, and the Primary Care Physician. Participates in patient care conferences as needed. Follows universal precautions, infection control procedures, and safety/sanitation standards of practice. Provides leadership for Home Health Aides by instructing, supervising, and evaluating care provided. Provides patients with appropriate nursing treatments as directed by the primary care physician. Thoroughly and completely records all assessments, treatments, and patient responses in the appropriate charting system to ensure continuity of care, maintenance of accurate legal documentation, and delivery of professional services. Provides patients and their families with education on appropriate medical self-care skills. Ensures that equipment and other necessary items and/or services are made accessible for patients. Assesses and monitors patients’ living environments to ensure they are clean, safe, and supportive of care needs. Participates in on-call duties as directed by the Home Health Administrator/Director of Nursing. Qualifications Must possess a current, unrestricted Registered Nurse (RN) license in the State of Kansas Must have current CPR certification. Prior home health experience is preferred. Must have a reliable method of transportation and a valid State driver’s license. Ability to communicate effectively in English, including reading, speaking, and writing, when performing duties that require this need, and with individuals from diverse backgrounds. The physical demands described below are representative of those that must be met by an individual to successfully perform the essential functions of this job. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential functions. Maximum occasional lift of 50 lbs. from floor to waist level; maximum occasional lift of 10 lbs. from floor to shoulder; maximum occasional carry of 5 lbs. for 100 ft.; maximum occasional push/pull horizontal of 40 lbs. at 36 in height for 100 ft.; maximum frequent push/pull horizontal of 10 lbs. at 46 in for 100 ft.; maximum occasional grip force of 40 lbs.; maximum occasional lateral pinch force of 15 lbs.; maximum occasional climb of 8 in; frequent forward reach; frequent bend; frequent fine motor coordination; occasional foot coordination; occasional balance; frequent sitting; continuous standing; and continuous walking.

Requirements

Must hold a current, unrestricted RN license in Kansas and a valid CPR certification. A valid driver's license, reliable transportation, and prior home health experience are required or preferred.
