The LPN supports the medication administration program and conducts resident assessments to ensure safety and compliance. They act as a liaison between residents and primary care providers while collaborating with the clinical team to manage care plans.
Candidates must hold a current and valid LPN license in the state of Colorado. Prior experience in assisted living or long-term care and proficiency with EHR systems are preferred.

The LPN will be a vital member of our clinical team, which includes an additional LPN, the Resident Life Director, Clinical Manager, Associate Executive Director, and this role. This position will report directly to the Associate Executive Director and partner with all clinical team members. The LPN will not have direct reports but will be instrumental in ensuring the highest standard of care for our residents. The LPN will also participate in stand-up or weekly operations meetings as determined by the Associate Executive Director.
