The Hospice RN assesses physical and emotional needs to create and implement patient-centered care plans. Responsibilities include administering treatments, managing pain, and collaborating with an interdisciplinary team to support patients and families.
As a Hospice Registered Nurse You will: Assess/monitor physical, emotional, and psychological needs of patients Create hospice care plans that align with the patient's wishes and goals Direct nursing care: administering medications, treatments, and interventions Provide pain and symptom management Educate and support the patient’s family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch
