About the role

Support assigned residents with routine daily activities and nursing care according to their assessment and care plans. Responsibilities include recording vitals and ADLs, monitoring resident safety, and reporting condition changes to supervisors.

Northbrooke Post-Acute, a 120-bed Skilled Nursing Facility, is located in Jackson, Tennessee servicing patients with wound care, rehabilitation and skilled services. Our focus is to provide the utmost quality of care to all of our patients while maintaining a compliant and welcoming environment. Position Overview

The Certified Nursing Assistant role supports assigned residents with routine daily activities and nursing care and services in accordance with the resident’s assessment and care plan, and as may be directed by the supervisors.

Use the wristband or photo to identify residents before administering treatments, serving meals, etc., as necessary.

Record all entries on flow sheets (ADLs. I&Os, percentage of meals eaten, vitals, bowel & bladder) in an informative, descriptive and legible manner. Record additional entries as requested/required by the charge nurse.

Use only authorized abbreviations established by this facility when recording information.

Report all changes in the resident’s condition to the Nurse Supervisor/Charge Nurse as soon as practical.

Report all accidents and incidents observed as they occur.

Transports residents to assigned locations for activity.

Monitor and supervise residents at all times to ensure resident safety.

Assist in passing meal trays to residents at meal times. Ensure the correct tray is given to the correct resident.

Assist and participate in a hydration program.

Assist patients at meal times

Qualifications

  • High school diploma or GED.
  • Must be a licensed Certified Nursing Assistant in Tennessee.
  • Active BLS/CPR

Requirements

Candidates must possess a high school diploma or GED and be a licensed Certified Nursing Assistant in Tennessee. An active BLS/CPR certification is also required.
