We're seeking an experienced Hospice Registered Nurse (RN) who believes that care goes beyond clinical skill - it's about compassion, connection, and preserving dignity at every stage of life. In this role, you'll deliver exceptional, patient-centered care and guide families through one of life's most important journeys.

Our hospice team provides individualized support that addresses not just physical symptoms, but also emotional, social, and spiritual needs - ensuring patients and their loved ones feel truly cared for.

What You'll Do

Deliver skilled, compassionate hospice care according to each patient's individualized plan.

Conduct thorough assessments and collaborate with the interdisciplinary group (IDG) to build and adjust care plans.

Educate patients and families on the end-of-life process, comfort measures, and available support services.

Monitor patient conditions, report changes promptly, and coordinate adjustments with the attending physician.

Supervise Hospice Aides and LPNs to ensure consistent, high-quality care.

Maintain accurate documentation in compliance with all state, federal, and accreditation standards.

Participate in IDG meetings, quality initiatives, and staff development opportunities.

Provide insight and emotional support to families, offering calm and guidance when it's needed most.

Support patients holistically - medically, emotionally, and spiritually - to enhance comfort and quality of life.

What You'll Bring

Graduate of an accredited nursing program.

Current Registered Nurse (RN) license in Illinois.Minimum of 1 year of RN experience; hospice experience preferred.

Excellent clinical judgment, assessment, and problem-solving skills.

Strong communication and organizational abilities.

Valid driver's license, reliable transportation, and current insurance.

A genuine commitment to compassion, integrity, and patient dignity.
