- Job Summary: ● A Licensed Physical Therapist professional who provides coordinated care to patients, evaluates patients and restores or maintains patient well-being according to the plan of care ● Evaluate and provide a treatment plan for a patient with physical, psycho- social and/or cognitive impairments ● Provide intervention to the deficit and elicit responses ● Collaborates with interdisciplinary team Job Qualifications: ● Education: Bachelor of Science in Physical Therapy ● Licensure: Current Texas State License as a Physical Therapist Assistant Experience: ● 1 year of minimum experience as a Physical Therapist in Home Healthcare or Acute care or Rehabilitation setting Skills: ● Therapy skills as defined as generally accepted standards of practice. ● Good communication and interpersonal skills. ● Proof of current CPR and Hepatitis profile. ● Knowledge of durable medical equipment ● Spanish is a Plus Essential Functions: ● Perform initial and ongoing clinical assessments to identify patient needs, determine the level of function at appropriate time points and the appropriateness of patient for Home health ● Develop and revise a plan of care in consultation with the physician and other care team members ● Provide therapeutic treatments, evaluate need for and use of adaptive equipment and environmental modifications, fabricate and maintain adaptive equipment and hand splints as applicable ● Evaluate outcomes of care with appropriate discharge planning. ● Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease ● Accept clinical assessments that are consistent with education and competence to care for patients. ● Supervise PTA as needed ● Complete appropriate documentation in a timely manner ● Meet mandatory continuing education requirements of the agency/license board ● Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate ● Demonstrate commitment, professional growth, and competency ● Promote Agency philosophy and administrative policies to ensure quality of care
