Manage a caseload of home health patients by performing comprehensive assessments and providing skilled nursing care. Coordinate with physicians and interdisciplinary teams to implement and monitor patient care plans.

Your Role Manage Your Caseload: Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges. Skilled Care: Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
Empower patients and families to manage their conditions safely at home.
Maintain accurate, timely clinical notes on company-issued iPads.

Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
Valid California Driver’s License and reliable transportation are mandatory.
Strong clinical judgment, organization, and the ability to work independently.
