Provide outpatient palliative care focusing on symptom management and advance care planning for patients with complex illnesses. Collaborate with interdisciplinary teams to coordinate transitions between care settings and ensure hospice readiness.
Must be a licensed Nurse Practitioner with national certification and DEA registration. Mandatory Hospice and Palliative Board Certification (ACHPN) is required.
