Responsibilities
Provide skilled nursing care in accordance with the patient's individual Plan of Care. Collaborate with the Interdisciplinary Team (PT, OT, ST, MSW) to ensure holistic outcomes. Supervise Licensed Vocational Nurses (LVNs) when assigned. Evaluate patient progress and report changes to the Case Manager or Director of Nursing. Complete documentation in a timely manner using our AI-powered charting software.
Requirements
Current California RN License. 1+ year of nursing experience (Home Health preferred). Valid Driver's License and reliable transportation. Compassionate, self-driven, and ready to work independently.
