The LVN provides medical care, implements nursing care plans, and maintains medical records for residents in a supportive housing facility. Responsibilities include collaborating with multidisciplinary teams, managing medications, and responding to medical emergencies.
Candidates must be a graduate of an accredited vocational nursing program and hold a current LVN license and BLS certification. Experience working with homeless or incarcerated populations is highly desirable.
Job Duties: Collaborate with housing staff to facilitate resident enrollment in medical benefits, connection with medical home, and other social services Collaborate with resident’s Primary Care and other professional disciplines to ensure effective care delivery and achievement of desired outcomes Ensure residents are provided with appropriate information to make informed choices related to treatment and that care reflects resident preferences Maintain communication with Homekey partner pharmacies, facilitate safe medication use on site in accordance with individual resident needs Ensure adherence to infection control standards on site among staff and residents In collaboration with housing agency, ensure daily wellness of residents Identify and respond to immunization needs of residents Plan and execute population health interventions among residents and staff that contribute to facility safety and health Ensure that clinical documentation is current, timely, and accurate Maintain confidentially in accordance with the Health Insurance Portability and Accountability Act (HIPAA) Utilize and contribute to county data platforms such as the Community Health Record, Salesforce platform, and others Recognize and respond to medical emergencies on site, follow up with MD communication or 911 activation Facilitate post-hospital transition plan of care for residents discharged from Emergency Department or inpatient hospital stays Plan and execute interventions to reduce number of ambulatory-sensitive Emergency Department visits Practice harm reduction principles on site, including interventions that minimize harmful effects of substance use while treating all residents with dignity and respect Other duties and responsibilities as required Minimum Required Skills & Qualifications: Graduate from an accredited vocational nursing program Hold current license as Licensed Vocational Nurse through Board of Vocational Nursing and Psychiatric Technicians Current Basic Life Support Certification
Experience with homeless or incarcerated populations is highly desirable Physical Requirements: Occasional sitting or stationary work Frequent standing, walking, kneeling, stooping, and ascending stairs Occasional lifting and carrying up to 20 lbs Frequent pushing and pulling Frequent reaching high, low, and level Work Environment: In-person onsite at a supportive housing facility.
