Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children.
The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices.
After Hours RN (DuPage Territory-Weekend) The After Hours RN is responsible, as a member of the interdisciplinary team, for the provision of skilled nursing assessment and hospice care to maximize the comfort and health of patients and families/caregivers consistent with Lightways Hospice and Serious Illness Care policies and procedures. The RN plans and directs services to meet the needs of individuals and families/caregivers within their homes and communities.
Provides after-hours skilled nursing care, assessments, and interventions to hospice patients in their homes, ensuring comfort and quality care Completes comprehensive patient evaluations, develops and updates care plans, and collaborates with the interdisciplinary team Responds to urgent patient needs, including medical and emotional crises, and communicates with physicians, triage, and care teams Administers medications, educates patients and families, and documents all care accurately and timely Maintains compliance with regulations, completes required training, and supports overall continuity and coordination of patient care Monday- Friday with weekend and holiday rotation.
Current RN license Minimum of 3 years RN experience in home health, critical care, or long-term care Valid driver's license and insured, reliable vehicle
