We are seeking a professional to join our team as a Service Coordinator for the Children's Long-Term Support (CLTS) Waiver Program. As the Service Coordinator, you will provide case management and service coordination services to families and individuals needing counseling and referral services for one or more of the following: intellectual and/or developmental, physical, or mental health disabilities.

Waukesha County Human Services building. Local travel is required for client home visits, with mileage reimbursement. Service Coordinators have the opportunity to work hybrid/WFH after six months.
Monday-Friday 8:00am-4:30pm, with occasional evenings to accommodate families' schedules.
$24.00 per hour
