Provide direct care and assertive outreach to individuals with psychiatric needs to help them live independently in the community. Responsibilities include assisting with daily living activities, transporting participants to appointments, and administering medication.
Requires a high school diploma and fluency in both English and Spanish. A bachelor's degree and at least one year of experience working with the target population are preferred.
The Center for Human Development, (CHD) is seeking Outreach Workers for our Adult Community Clinical Services (ACCS), located in Springfield, MA. ACCS is funded through the Department of Mental Health and is a multidisciplinary outreach approach for persons served in need of psychiatric and other services living independently in the community. This involves active engagement including assertive outreach, consistent assessment and treatment planning, skill building, symptom management, addiction treatment, and behavioral and physical health monitoring and support.

ACCS teams include licensed clinicians, RNs, housing specialists, substance use specialists, direct care staff, and peer/family specialists. The Team works collaboratively to provide person-centered, evidence-based services in support of Persons’ treatment goals. The Direct Care Staff assists participants in need of psychiatric services acquire the skills necessary to live independently in the community. S/he works within the Agency’s team model, establishing open communication, support, and accountability among co-workers.
Pay Rate: $20/hr
