The Home Care RN is responsible for traveling to patients' homes to perform assessments, administer treatments, and manage individualized care plans. They coordinate with physicians and supervise home health aides to ensure continuity of care and patient independence.
Candidates must be a graduate of an accredited nursing school with current state licensure, CPR certification, and a valid driver's license. 1-2 years of patient-centered care experience is required, while a Bachelor's degree in nursing is preferred.

A Home Health Nurse, or Home Health RN, is responsible for traveling to a patient’s home to administer their services and helping patients maintain their independence. Their duties include administering at-home IVs, changing dressings, cleaning wounds, and updating Doctors about their patient’s health.
We offer Full-Time, Part-Time, Baylor, and Per Diem hours with excellent salary and benefit packages. While home care experience is beneficial, we provide training for the right individuals. Strong clinical support and educational resources are also available.
Perform initial and ongoing patient assessments to determine home care needs. Develop and revise individualized care plans based on patient assessments. Administer medications and treatments as prescribed. Provide health care instructions to patients and caregivers. Facilitate discharge planning to ensure continuity of care. Regularly re-evaluates patient nursing needs.
Prepare clinical notes and update the primary physician regularly. Communicate effectively with physicians and community health resources to coordinate care plans. Supervise and evaluate home health aide care every fourteen days. Ensure that arrangements for equipment and other necessary items and services are available. Promote health and independence across the lifespan through our skilled services and education of patients, families, and caregivers.
Graduate of an accredited school of nursing. 1-2 years of recent patient centered care/nursing experience. Current state licensure, CPR certification, and a valid driver’s license. Bachelor's degree in nursing with one year of home health care experience preferred. Strong observation, communication, and problem-solving skills. Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.

Physical Requirements Prolonged or considerable walking or standing. Ability to lift, position, or transfer patients (up to 50 lbs.). Frequently reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.
Supervisory Responsibilities Supervise and coordinate care with LPNs and LNAs.
