We’re seeking a Home Health Social Worker (MSW) to provide emotional, social, and practical support for patients and their loved ones.
You’ll be part of an interdisciplinary team that helps individuals navigate serious illness, life transitions, and the complexity of home-based care.
What You’ll Do as a Home Health Social Worker (MSW): Conduct psychosocial assessments to identify patient and family needs and develop care goals aligned with the individualized Plan of Care.
Deliver emotional support, counseling, and crisis intervention services to patients and caregivers.
Educate families on available community resources, advance directives, care planning, and coping strategies.
Collaborate closely with nurses, therapists, physicians, and other members of the care team to ensure comprehensive, patient-centered care.
Provide support and guidance through serious illness, loss, grief, and end-of-life decisions.
Communicate changes in patient or family condition that may impact care planning, safety, or psychosocial well-being.
Document all services and interventions thoroughly, accurately, and in accordance with regulatory and agency standards.
Advocate for the dignity and rights of patients and families, while offering sensitivity, respect, and culturally competent care.
