• You’ll Do:
• Provide 1:1 and group support based on individualized service plans Help individuals develop skills in communication, self-care, and daily living Implement care plans developed by clinical teams (OT, PT, Speech, Nursing, Behavioral) Track progress and complete clear, professional documentation Support community engagement, including transportation and activities when needed Assist with hands-on care needs, including mobility and dietary support Collaborate with team members, families, and community partners Serve as a positive role model, promoting safe and respectful environments

Basic knowledge of Microsoft Word and the internet. Successful completion of all state and agency mandated trainings. Must be willing to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies. LANGUAGE SKILLS Must be fluent in English and have the ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to communicate effectively with individuals under supervision, employees of the organization, families, and members of the community.
