Provides case and group work services to patients and their families during hospital stays to maximize functioning. Ensures continuity of care through referrals to appropriate follow-up services and advocacy for healthcare access.
Requires a Master's Degree in Social Work and an LMSW license within one year of employment. Must have at least 6 months of post-masters experience with adult populations in a psychiatric setting.
Provides case and group work services to patients and/or their families during patient's Hospital stay. Ensures continuity of medical care by referral to appropriate follow-up services.
Master's Degree in Social Work from an accredited school of social work.
LMSW required within one (1) year of employment.
Current Driver’s License and/or own transportation available to maintain caseload, schedules and travel to clients’ homes, if required by site.
