Per Diem Community Health Nurse - LHCSA

Position Summary

Provides skilled nursing care to patients in their home, including nursing assessment, interventions and supervision of Home Health Aides and Personal Care Aides in the field on a per visit basis.

Principal Responsibilities

• Per diem RN Field Supervisor position available with a well-established Licensed Home Care agency in Nassau County. Duties include Patient Assessment, Reassessment and Aide supervision for patients receiving personal care and NHTD services.
• Perform in-home client assessments and/or RN supervisory visits for home care aides
• Complete and submit all agency paperwork in a timely manner regarding visits to meet NYSDOH requirements
• Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Adheres to the organization’s policy in regards to absenteeism and appearance

Salary Range

$55-60 per /hour, $75-4100 per visit, commensurate with experience.

Job Competencies &

Minimum Qualifications

• Graduate of an accredited school of nursing; Bachelor’s degree (B.A.) in Nursing from a four-year college or university preferred. One year of hands on nursing experience.
• Must have current license and registration to practice as a Registered Professional Nurse in New York State. One year Home Care experience preferred.
• Problem solving abilities while multi-tasking; ability to exercise sound judgment
• Excellent verbal and written communication skills
• Excellent customer service and organizational skills

Working Condition/Physical Demand

• Experience with a Clinical EMR, HHA Exchange experience preferred
• Must have a car and be willing to drive to Long Island client homes
• This is fully field work role
