The CCSS Worker provides community-based rehabilitative services to individuals with behavioral health and substance use disorders to improve functional abilities and independent living. Responsibilities include delivering strengths-based interventions, assisting with life skills, and collaborating with a multidisciplinary treatment team.
The candidate should have mid-level experience in providing community support services and behavioral health interventions. Proficiency in clinical documentation and the ability to conduct ongoing client assessments are required.
The CCSS Worker provides community-based, rehabilitative support services to individuals with behavioral health and substance use disorders. Services are delivered in real-world settings and are designed to improve functional abilities, promote recovery, and support independent living. The position emphasizes skill-building, goal attainment, and integration into the community in alignment with the client’s individualized treatment plan. Key Responsibilities The CCSS Worker delivers structured, strengths-based interventions that address functional impairments across multiple life domains. This includes assisting clients with developing and maintaining independent living skills such as personal hygiene, medication adherence, budgeting, transportation use, and housing stability. The worker supports clients in accessing and maintaining employment or educational opportunities by building job readiness skills, assisting with applications, and reinforcing workplace expectations. The role includes facilitating socialization and interpersonal skill development through coaching, modeling, and guided practice. The CCSS Worker assists clients in building healthy relationships, improving communication, and establishing appropriate boundaries. Recreational and community engagement activities are used to reinforce pro-social behavior and reduce isolation. The CCSS Worker conducts ongoing assessments of client strengths, needs, and barriers, and uses this information to support individualized goal progression. Services are delivered in collaboration with the multidisciplinary treatment team, including therapists, case managers, CPSWs, and medical providers. The worker participates in treatment planning, team meetings, and care coordination efforts. Accurate and timely documentation is required for all services provided. Documentation must reflect interventions delivered, client response, progress toward goals, and continued medical necessity. The CCSS Worker monitors for changes in mental health status, substance use risk, and environmental stressors, and communicates relevant updates to the treatment team.
