The opportunity As a Social Worker / Case Manager in Nevada County for Recover, you’ll work directly with patients to help them with challenges and support their recovery.
You will closely collaborate with our medical team to connect patients to medical care, and will be expected to develop relationships with your patients and help guide them through their recovery journey.
You will also collaborate regularly with the rest of the treatment team consisting of the medical director, physicians, nurse practitioners, and case managers to develop and execute comprehensive treatment plans for patients.
This position is best suited for someone who is comfortable working independently, will take ownership of patient care, build rapport, and who works well in a collaborative team environment.
Must be comfortable with field-based services (e.g., working with people living on the street).
This role is full time.
You will report to Recover’s Clinical Supervisor.
Salary is commensurate with experience and qualifications, and the position includes benefits.

Role is 40 hours / week across 5 days.
