How you’ll make an impact in this role Orchestrate Comprehensive Care Plans

• Lead the implementation and continuous monitoring of personalized care strategies to ensure every patient reaches their recovery milestones.

• Coordination: Act as the central point of contact for the entire care team, synchronizing specialists and support staff to provide a seamless, holistic patient experience.

• Transitions: Evaluate and coordinate discharge needs early in the stay, working closely with the healthcare team to ensure patients have everything they need for a successful return home.

What minimum requirements you’ll need Licensure / Certification / Registration

BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted. Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education

Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire required.
