The RN Care Coordinator provides nursing and coordination services to support patient-centered care, chronic disease management, and population health in a community health setting. Responsibilities include identifying care gaps, facilitating follow-ups, and documenting patient interactions within the EPIC EMR system.
Candidates must be a Registered Nurse with a valid Colorado license and current BLS/CPR certification for Healthcare Providers. Experience in care coordination or chronic disease management is preferred, along with a valid driver's license and auto insurance.

Job-Specific Responsibilities Provide registered nursing services and care coordination to support clinical, preventive, chronic, and population health care needs across the lifespan. Support care coordination activities across clinical, preventive, chronic, and population health initiatives, including identification of care gaps and facilitation of appropriate follow-up per protocol or provider direction. Assist with patient engagement, education, and communication related to health maintenance, chronic conditions, preventive services, and care planning within RN scope of practice. Document nursing care, patient interactions, and coordination activities accurately in EPIC, the Electronic Medical Record (EMR), and utilize EPM/EHR systems according to established workflows. Respond to urgent clinical communications, including critical laboratory results and provider requests, during scheduled work hours. Clinic and Organizational Responsibilities Perform all duties in alignment with the philosophy, mission, values, and Communicate with Heart service model supported by the Board of Directors of Pueblo Community Health Center, Inc. Adhere to all Pueblo Community Health Center policies and procedures, including Infection Control and Nursing Policies and Procedures. Collaborate effectively with providers, Medical Assistants, Care Coordinators, and other members of the integrated care team to promote quality, continuity, and efficiency of care. Support clinic operations and patient flow, including assisting with patient intake, scheduling, messaging, record management, and administrative tasks as assigned. Participate in employee health activities and other organizational clinical services as assigned. Attend mandatory meetings, trainings, and required education. Maintain current RN licensure and CPR certification. Provide assistance in emergency situations when providers are called upon to render care to individuals near the health center, as appropriate. May provide supervision or support to students or trainees assigned to the health center. Perform other duties as assigned. The employee frequently moves about the office; and positions self while performing patient care tasks and routine office duties such as accessing files and operating equipment. The employee frequently moves or positions objects weighing up to 25 pounds and occasionally up to 100 pounds while performing patient care tasks and routine office duties such as opening file drawers, moving patient files, etc.
