The Specialty Case Manager provides community-based case management, planning, and advocacy for high-risk individuals with mental health or substance use diagnoses. They are responsible for developing individualized treatment plans and connecting patients to essential health, housing, and social services.
A high school diploma or GED is required, while an associate degree in behavioral health and peer certification are preferred. Candidates should ideally have 1-3 years of experience in behavioral health outreach and working with special populations, along with a valid AZ driver's license.
