The Ambulatory Care Nurse provides nursing care management and coordinates patient plans of care to maximize community-based living. They act as a primary communication link between physicians, families, and interdisciplinary healthcare teams.
Candidates must be a Registered Nurse with at least 5 years of nursing experience. An Associate's degree in Nursing is required, while a Bachelor's degree is preferred.
