The Ambulatory Care Nurse provides nursing care management and coordinates patient plans of care to maximize community-based living. They act as a communication link between physicians, patients, families, and interdisciplinary healthcare teams.
Candidates must be a Registered Nurse with either a Diploma in Nursing or an Associate's Degree in Nursing. No prior professional experience is required for this position.
