Provide high-quality hospice and palliative nursing care to patients in their homes and at the Hospice House. Responsibilities include performing skilled nursing care, managing symptoms, and coordinating with physicians and team members.
Must hold a current South Carolina Registered Nurse license and have at least 2 years of experience in acute hospital or home care settings. Candidates must be skilled in crisis intervention and pain management while being able to work independently.
RECEIVE FULL-TIME PAY FOR PART-TIME WORK~ POSITION IS CONSIDERED FULL-TIME WITH BENEFITS Summary We are seeking a compassionate and skilled Home Care Nurse to provide high-quality hospice and palliative care to patients in their homes. The ideal candidate will have a strong background in patient care, have the ability to work independently and be able to work cooperatively with the Home Care Clinical Director and other team members to effectively meet patient care goals. Work Schedule 4:30 PM Friday to 8:00 AM Monday
Current South Carolina license as a Registered Nurse. Minimum 2 years' experience as a staff nurse in an acute hospital setting or Home Care experience. Ability to assess and respond to the complex needs of patients and families in the home setting. Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Skilled in pain and symptom control management and crisis intervention. Self motivated individual with the ability to prioritize effectively. Ability to function as part of a team. Ability to embrace the mission and culture of the organization.
Visits patients served by Hospice and Palliative Care of the Piedmont, within its service area, to provide nursing care, instruction, and support to patients and their families. Performs skilled nursing care as ordered according to the plan of care. Visits will include: On-call visits as indicated by the nurse's assessment of information received (i.e. pain and/or symptom management, crisis intervention, death). Scheduled visits for unstable patients (i.e. visit actively dying patients who require extra interventions and/or support, scheduled treatments). Complete patient intake and admission visits (i.e. new referrals that must be seen on weekends). Bereavement visits in accordance with the plan of care. Maintains effective communication with other team members for patient updates. Obtains patient updates and assignments prior to beginning of shift. Reports to appropriate staff person at end of assigned shift. Communicates patient status with physician in order to obtain necessary MD orders. Documents patient and family contacts, nursing care provided, and other charting requirements in an accurate and timely manner (within 24 hours of visit or call). Implements or assists with admission procedures to the Hospice House. Audits charts and assists with other activities as requested. Assist Hospice House with nursing related duties, if needed. Performs other related duties as necessary.
