Must hold a current California Physical Therapist license and be a graduate of an accredited Physical Therapy program. Experience in skilled nursing or post-acute care is preferred.

Sunnyvale Gardens Post Acute is seeking a PRN Physical Therapist to join our skilled nursing and post-acute care team. The ideal candidate is compassionate, clinically strong, and committed to helping patients restore function, mobility, and quality of life. Responsibilities Evaluate patients and develop individualized treatment plans Provide therapeutic exercise, gait training, balance training, and functional mobility interventions Document evaluations, treatments, goals, and progress in accordance with regulatory standards Collaborate with an interdisciplinary care team to optimize patient outcomes Educate patients and families on treatment plans, safety, and home exercise programs Maintain compliance with state, federal, and facility policies Qualifications Current California Physical Therapist license in good standing Graduate of an accredited Physical Therapy program Experience in skilled nursing or post-acute care preferred Strong communication, clinical, and documentation skills Ability to work independently and as part of a team Schedule & Compensation PRN / Per Diem (flexible scheduling) $57–$67 per hour, based on experience Why Sunnyvale Gardens Post Acute?
