Provide direct, community-based behavioral health services and conduct comprehensive clinical assessments for individuals in stabilization. Develop individualized stabilization plans and coordinate care with multidisciplinary teams and community partners.
Requires a Master's degree and a valid LMHP or LMHP-S license. Candidates must have at least 2 years of clinical experience.
Flexible schedule Opportunity for advancement Training & development The In-Person Clinician provides direct, community-based behavioral health services to individuals enrolled in Community Stabilization. This role requires clinical expertise, strong collaboration skills, and the ability to work effectively within a multidisciplinary team in high-acuity environments.
Provide a minimum of 12 hours per week of direct, face-to-face community-based services, delivered in the client’s natural environment in collaboration with a QMHP.
Conduct comprehensive clinical assessments to determine service needs, risk factors, and stabilization planning.
Develop and implement individualized stabilization plans in coordination with clients, case managers, and crisis staff.
Facilitate care coordination across providers, including internal clinical teams and external community partners.
Maintain consistent communication with staff to ensure service quality, compliance, and accurate documentation in accordance with agency and regulatory standards.
Complete timely, thorough, and clinically sound documentation that reflects services rendered and client progress.
Initiate referrals and coordinate linkages to additional behavioral health, medical, housing, or community resources as clinically indicated.
Collaborate with interdisciplinary professionals to develop integrated diagnoses and treatment approaches that support long-term stabilization and recovery.
