The LPN is responsible for monitoring health, self-care, and personal hygiene needs for individuals in medical frail homes. Key duties include providing direct hands-on support, administering medications, and performing routine hospital check-ins.
Candidates must be a licensed and registered LPN by the New York Education Department and possess a valid NYS Driver's License with reliable transportation. Prior experience working with individuals with developmental disabilities is preferred.
What does a Licensed Practical Nurse do? A Licensed Practical Nurse (LPN) will be responsible for monitoring the people we support in our medical frail homes as it relates to health, self-care and personal hygiene needs. This position will assist with hands on direct support as well administering medications and performing routine hospital check ins for the people we support.
