Overview: Case Manager Connect, develop, and maintain a strong, trusting relationship with the families served and children & young people placed to foster engagement and support growth. Conduct regular home visits to monitor the child’s progress and well-being Develop and implement individualized care plans. Coordinate with natural families, resource families, and other service providers to promote open communication, collaboration, and to ensure comprehensive care for the children. Conduct regular meetings with children & young people, caregivers, and professionals in a variety of settings- homes, community spaces, professional offices. Maintain detailed, accurate, and timely documentation to ensure safety and provide effective tracking and reporting. Provide on-call coverage assisting families through crisis intervention transportation.

Requirements: This position is primarily located in McKeesport. All positions are also expected to come to training, meetings, and other events at Main Campus in McKeesport, as scheduled. The travel required for this position is frequent.

Valid PA driver’s license, vehicle, registration, and proof of insurance. Requires frequent travel throughout greater Pittsburgh Area, including transportation of young people, with use of all types of car seats. 21 years of age or older. Physically staying fit and able to work with children along with free of all communicable diseases. Requires working varied hours, including evenings or weekends.
