$35 - $40 / HOUR
Responsible for conducting hospice and palliative care visits, including admissions and discharges, in home and community settings. The role involves implementing care plans, providing patient education, and collaborating with an interdisciplinary team.
Requires a nursing degree, a current active RN license, and at least one year of nursing experience. CPR certification and a valid driver's license are mandatory, while CHPN certification is preferred.
The Hospice of Greater Cleveland (partnership between McGregor & Judson) delivers compassionate, patient-centered care to individuals receiving hospice services and provides emotional support to families during the end-of-life process. Under the direction of new leadership, the Hospice RN will contribute to a culture focused on compassionate care, teamwork, operational excellence, accountability, and continuous quality improvement.
As a Hospice Nurse, you will be responsible for completing hospice and palliative care visits in the home setting (which includes the community or agency setting) as assigned. These visits may include admission, routine, recertification, discharge and/or death visits. Follows the established plan of care to provide care, treatments, and education. Demonstrates knowledge and support of the philosophy, purpose, and goals of hospice care as well as those of McGregor and its policies.
