The Nurse provides holistic nursing support, patient care, and medication management for men experiencing homelessness. Responsibilities include performing primary nursing procedures, coordinating with a multidisciplinary care team, and managing patient admissions and discharges.
Candidates must be a graduate of an accredited nursing school and hold a valid Registered Nurse (RN) license in DC. Current CPR certification is required, and two years of experience in an acute care setting is preferred.
Rotating shifts weekdays and alternating weekends (7:00am-3:30pm, 11:00am-7:30pm, 7:00am-7:30pm). Occasional night shift availability required. Full-time position is 32-40 hours per week.
In collaboration with clinical and support staff, help manage medical issues and coordinate with the care team to address the multiple issues that contribute to homelessness. Administer medications as ordered by providers and document appropriately in MAR. Transcribe new orders in patient charts and integrate them into patient care plan. Work with clinical unit staff, including Respite Care Assistants, Providers, and Unit Clerk to effectively deliver high-quality care to patients. Teach and encourage patients' self-care practices regarding care of their illnesses, management of their medications, and activities of daily living. Perform primary nursing procedures to include patient assessment, wound care, glucometer testing, insulin administration, vital signs interpretation. Admit new patients to the respite unit and conduct discharges. Prepare patient referrals to hospitals and clinics, and process information received on referrals. Communicate pertinent information to the Provider-on-call, Provider, and Chief Nursing Officer as appropriate. Assure an accurate change of shift report is provided. Chart appropriate information in progress notes on a regular basis. Other duties as assigned.
Graduate of an accredited school of nursing. Licensed in DC as a Registered Nurse (RN). Current CPR certification. 2 years' experience in acute care setting preferred.
