Provides individual and group social work services to patients and families in healthcare settings using psychosocial assessments. Formulates safe discharge plans and coordinates with interdisciplinary teams and community resources to maximize patient functioning.
Requires a Master of Social Work and a current license to practice as a Master or Clinical Social Worker in New York State. Candidates must have 1-3 years of relevant professional experience.
Provides individual, and/or group work services to patients and/or their families in various health care settings. Utilizes psychosocial assessment skills to deliver social work services.
