The RN Clinic provides direct patient care, care coordination, and health education within an ambulatory community health setting. Responsibilities include performing nursing assessments, managing chronic conditions, and collaborating with an interdisciplinary team to ensure quality outpatient care.
Candidates must have an associate degree, a current unrestricted Oregon RN license, BLS/CPR certification, and a valid driver's license. A BSN and at least two years of clinical experience in acute or ambulatory care are preferred.
The RN Clinic role is a licensed registered nurse who delivers direct patient care, care coordination, and health education within an ambulatory community health clinic setting. This role collaborates with providers and the interdisciplinary team to assess, plan, implement, and evaluate care for patients across the lifespan. The RN Clinic role is responsible for performing clinical procedures, managing acute and chronic conditions, and supporting population health initiatives. The position emphasizes patient advocacy, evidence-based practice, and a commitment to quality and safety in the outpatient environment.
Minimum two years of RN clinical experience in acute or ambulatory care preferred. Care coordination experience in an ambulatory setting preferred. Experience in nurse-led patient visits preferred (i.e. diabetes and chronic disease education, anticoagulation management)
Pediatric RNCC: Experience working with pediatric patients preferred
