The Case Manager provides essential support to patients and families by planning, coordinating, and monitoring access to health services, housing, and employment. They are responsible for assessing patient needs, developing individualized treatment plans, and implementing interventions for high-risk individuals.
A high school diploma or GED is required, while an associate degree in a behavioral health field is preferred. Candidates must have 1-3 years of experience with GMH/SU populations and outreach activities, along with a valid AZ Driver's License and Fingerprint Clearance card.
