Conduct in-depth face-to-face assessments to develop and implement individualized care plans for members. Coordinate medical, behavioral, and community-based services while advocating for high-quality care access.
Requires a current unrestricted Virginia RN license and at least 3 years of clinical experience. Candidates must reside in or near Norfolk, VA, and be able to travel up to 75% of the time.

Effective communication skills, both verbal and written. Education Associate degree in Nursing required Bachelor’s degree preferred Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $60,522.00 - $129,615.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
