Provide comprehensive medical case management services to Cornerstone Family Healthcare patients living with HIV (PLWH).
Provide comprehensive case management services to PLWH. Provides a client-centered service that links patients with health care, psychological and social services in a manner that ensures timely, coordinated access to medically appropriate levels of care and support services and continuity of care. Develops linkages and networks with other groups and agencies offering services to PLWH to help promote cooperation and reduce duplication of services among agencies. Establishes peer support groups and focus groups for our patients, and PLWH referred from other agencies. Provide crisis intervention to patients. Provides follow-up and outreach for those patients not engaged in care. Provide documentation to reflect patient interaction, outreach attempts service plan updates, and biannual psychosocial assessments. Performs updates and maintenance of patient charts. Provides case management input and psychosocial information concerning patients during medical conferences. Enter case management data into EMR and AIRS databases. Attends conferences and various training sessions, as needed. Refers patients to programs, support groups and medical treatment, as needed. Develop relationships and represent Cornerstone Family Healthcare in community organizations. Attend and participate in monthly department, All Staff and other required meetings. Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Cornerstone Family Healthcare Policy and Procedure Manual and Employee Handbook. Maintain confidentiality of all aspects of Cornerstone Family Healthcare including, but not limited to, patient confidentiality, financials, and employee relations. Perform other duties as assigned.
Must be Bilingual (English/Spanish) Bachelor of Science/Art in Social Work, Psychology, Human Services or related field. Highly organized with excellent oral and written communication skills. Ability to maintain a nonjudgmental disposition and communicate with a diverse population.
