The Physical Therapist is responsible for evaluating patients and implementing individualized treatment plans in accordance with physician orders. They also supervise Physical Therapy Assistants and collaborate with interdisciplinary teams to ensure high-quality resident care.
Candidates must possess a valid California Physical Therapy License and a current CPR certification. Experience in geriatrics or rehabilitation medicine is preferred along with strong interpersonal and communication skills.
The Staff Physical Therapist is responsible for evaluating and treating patients, collaborating with healthcare team members, documenting services, and participating in staff training and education. Key Responsibilities Evaluate patients within 24 hours (or 1 business day) of physician referral Develop and implement individualized treatment plans with physician approval Deliver treatment in accordance with physician orders and PT best practices Supervise Physical Therapy Assistants in accordance with state regulations Maintain detailed, accurate, and timely documentation of all patient care Participate in interdisciplinary meetings including Care Conferences and Utilization Reviews Provide caregiver education and training for post-discharge care Recommend and assist with ordering of durable medical equipment (DME) Ensure all department equipment is maintained in good working condition Conduct in-service education for facility staff Qualifications Valid California Physical Therapy License (Required) Current CPR Certification (Required) Experience in geriatrics or rehabilitation medicine preferred Strong communication, documentation, and interpersonal skills Ability to relate effectively with patients, families, and interdisciplinary team Reliable transportation and/or ability to relocate to Porterville, CA We Offer Competitive pay starting at $60/hour Full benefits package for eligible employees, including: 401(k) with employer match Medical, dental, and vision insurance Paid time off (PTO) Supportive team culture and ongoing training Opportunities for career growth and leadership If you're passionate about helping others regain their strength and independence in a caring, team-oriented setting, apply today to join Sequoia Transitional Care in Porterville.
