Provide one-on-one personalized care to patients in their homes, assisting with daily activities, hygiene, and meal preparation. Maintain patient records and build supportive relationships to ensure the mental and physical well-being of clients.
Candidates must possess a valid driver's license, car insurance, and CPR certification, and must pass a background check and TB test. Previous home care experience is preferred but not required.
You will be directly working with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role.
Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles Provide attentive personal care and companionship Care for patients by changing bed linens, doing laundry, cleaning the home, and assisting with personal hygiene and grooming Prepare meals and assist with feeding when needed Run errands such as grocery shopping, picking up prescriptions, and attending appointments Maintain records of patient care, condition, progress, or problems to report observations to the supervisor Build positive relationships with patients by conversing, reading aloud, or engaging in meaningful activities to keep them mentally healthy and alert Provide medication reminders Assist with mobility, transfers, and light exercises Monitor patient safety and overall well-being Assist with toileting and incontinence care when necessary Support patients with daily routines and independent living skills Escort clients to appointments or community activities when needed Provide light housekeeping and homemaker services to maintain a safe and comfortable environment Offer emotional support and companionship to clients and families
