Responsibilities:
Resident
Care: Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and oral hygiene. Help residents with mobility, including turning, positioning, transferring, and ambulating using assistive devices as needed. Feed residents or assist with meals, ensuring proper nutrition and hydration, and monitor food and fluid intake. Monitor and report changes in residents’ physical, emotional, or behavioral conditions to the nursing staff.

Signs and
Monitoring: Measure and record residents' vital signs, such as temperature, blood pressure, pulse, and respiration, as directed by nursing staff. Assist with monitoring residents’ skin conditions, reporting any signs of skin breakdown or pressure sores. Assist with weight checks, intake and output monitoring, and other health-related measurements.
Documentation
Accurately document care provided and residents' responses to care in the facility’s electronic health record (EHR) or paper charts. Maintain records of residents' food and fluid intake, toileting habits, and other observations as required. Report any concerns or incidents to the supervising nurse and complete required documentation.

Comfort: Ensure residents’ safety by adhering to facility protocols and infection control guidelines. Assist in maintaining a clean, organized, and comfortable living environment for residents. Practice proper hand hygiene, use personal protective equipment (PPE) as required, and follow all safety procedures.
