The Care Coordinator is responsible for assessing clients, documenting symptoms, and linking individuals with necessary community resources to support recovery. They maintain continuity of care in compliance with Florida statutes and manage treatment plan reviews and progress notes.
Requires a Bachelor's degree and one year of related case management experience with adult or child populations. Candidates must obtain AHCA and FCB certifications within specified timeframes and possess a valid Florida Driver's License.
Bachelor’s degree from an accredited college or university in any field.
Licensure is not required.
One year work experience providing at least twelve months of related case management/care coordination service to either the adult or child population. Must be able to assist clients struggling with multiple stressors. Understanding of the special problems and treatment of consumers with mental illness. Understanding of community resources and social support development is required.
Completion of AHCA approved targeted case management training within 3 months of hire.
Must be certified through the Florida Certification Board (FCB) within 6 months of hire.
Must be 21 years of age.
Negative Drug Screening prior to hire and throughout employment.
Clear Criminal Background Check throughout employment with Suncoast Center and local checks prior to hire and annually along with DCF Level II prior to hire and as required by DCF, and Must have valid Florida Driver’s License Motor Vehicle checks prior to hire and annually with acceptable driving record maintained throughout employment along with reliable vehicle, valid registration, and valid vehicle insurance. (must be in compliance with Florida Laws and Administrative Codes with fingerprinting and determined to be of good moral character).
Services are provided on-site in the office. Schedule may be irregular, based on client availability and must be available to work up to two evenings a week up to 6:30pm EST.
