Responsible for the clinical and financial evaluation of hospital and community referrals for admission and re-admission. Manages the entire process including insurance authorization, bed management, and coordination with families and care partners.
Responsible for clinical and financial evaluation of hospital and community referrals for admission and re-admission to St. John's Home, including all aspects of the process: screening, insurance verification and authorization, continuous communication with families & various community care partners, bed management, and oversight of the admission paperwork. Works in collaboration with other departments to provide support to residents and families, as well as maintain census and payer mix.
