Facilitate family team meetings using a wraparound model to develop coordinated plans of care for youth in the juvenile justice system. Collaborate with families and community partners to reduce system involvement and strengthen family functioning.
Requires a bachelor's degree or 2-4 years of experience working with high-risk youth in juvenile justice. Must have at least two years of experience in care coordination or direct services to youth and families.
