Provide rehabilitative physical therapy services to home health patients, including assessing functional status and implementing personalized plans of care. Collaborate with an interdisciplinary team to coordinate comprehensive care and document patient progress according to regulations.
Requires a Bachelor's or Master's degree in Physical Therapy and a valid Ohio state license. Minimum of 2 years of experience is required, along with CPR certification and a valid driver's license.
Provide physical therapy services to patients according to a plan of care. This may include assessing and evaluating therapeutic/rehabilitative/functional status and participation in developing the total plan of care as well as providing direct physical therapy treatment. Assess patients for muscle strength, mobility, gait, ROM and transfer capabilities. Instruct patients, families and caregivers in the use and care of therapeutic appliances. Report patient’s reaction to treatment or changes in care to patient’s physician as needed and appropriate. Initiate physical therapy programs and instruct other personnel, patient and/or family/caregivers in their therapy program, as well as the goals of therapy. Supervise the Physical Therapist Assistant (PTA) as necessary. Participate in Otterbein Universal Home Health Quality Assessment/Performance Improvement program. Confer with the attending physician, home care team members and patient and/or family/caregivers to promote coordinated and comprehensive care. Prepare and submit clinical and progress notes in a timely manner and in accordance with Otterbein Universal Home Health, state and federal guidelines and regulations. Education and Experience Education: Bachelor’s degree or master’s in physical therapy.
