Requirements
- Conduct psychiatric assessments and behavioral health evaluations within long-term care and skilled nursing environments
- Provide ongoing psychopharmacology management and psychiatric follow-up visits
- Evaluate and manage conditions including depression, anxiety, psychosis, mood disorders, dementia-related behaviors, chronic psychiatric illness, and behavioral disturbances
- Adjust and monitor psychiatric medications based on resident presentation and treatment response
- Develop individualized psychiatric care plans in collaboration with interdisciplinary teams
- Work closely with nursing staff, therapists, physicians, social workers, and facility administrators regarding resident care needs
- Support behavioral management strategies and continuity of psychiatric treatment within facility settings
- Maintain timely, organized, and compliant clinical documentation
- Communicate effectively with residents, family members, caregivers, and facility staff
- Participate in maintaining quality behavioral health services across assigned facilities
Qualifications
- Board Certified Psychiatric Mental Health Nurse Practitioner (PMHNP)
- Active and unrestricted New York State Nurse Practitioner license
- DEA registration preferred or ability to obtain
- Comfortable practicing independently within long-term care environments
- Prior exposure to geriatric psychiatry, behavioral health, skilled nursing, rehabilitation, post-acute, or nursing home populations preferred
- Experience managing dementia-related symptoms, chronic psychiatric illness, and psychotropic medication regimens strongly preferred
- Strong interpersonal, organizational, and communication skills
- Ability to work collaboratively within interdisciplinary healthcare settings
