Position Title

RN Case Manager Position Purpose: The RN Case Manager provides non-clinical case management services in support of the Vermont Army National Guard’s medical readiness mission. The RN Case Manager coordinates care for Soldiers with medical and dental readiness concerns, ensuring timely case tracking, documentation, referrals, and communication between Soldiers, providers, military leadership, and medical facilities. The position supports operational readiness and deployability through effective care coordination and medical readiness management.

Duties and Responsibilities

Establish and maintain eCase profiles for Soldiers referred by providers within one business day of referral. Coordinate and monitor Soldier medical treatment plans and referrals with military and civilian healthcare providers. Maintain accurate medical documentation and administrative charting in systems including eCase, MEDCHART, MEDPROS, MHA/PHA, AHLTA, MODS, HRR, and related Army medical readiness systems. Track medical readiness statuses, profiles, appointments, and follow-up documentation for Soldiers. Prepare Soldier records and supporting documentation for Medical Evaluation Boards (MEB) and Physical Evaluation Boards (PEB). Educate Soldiers on available medical readiness resources and privacy rights. Coordinate with unit leadership regarding Soldier medical appointments and readiness compliance. Upload, scan, verify, and maintain medical documentation in accordance with Army regulations and HIPAA requirements. Participate in Soldier Readiness Processing (SRP), Periodic Health Assessment (PHA), mobilization events, and monthly case reviews. Generate monthly reports regarding case statuses, profile management, and medical readiness metrics. Support mobilization operations and medical readiness events as directed by the Office of the State Surgeon. QualificationsRequirements: Active Registered Nurse (RN) license in good standing in the State of Vermont. Associate’s or Bachelor’s degree in Nursing from an accredited institution. A Bachelor of Science in Nursing (BSN) is highly preferred but not mandatory. A minimum of two (2) years of relevant clinical or case management experience. A minimum of two (2) years of supervisory or team lead experience in a clinical or case management setting. This experience must be in addition to the base clinical experience requirement (for a total of at least 4 years of professional nursing experience). Knowledge of HIPAA, medical documentation standards, and healthcare coordination processes. Experience with military healthcare systems, medical readiness, or case management preferred. Strong communication, organizational, and documentation skills. Ability to obtain and maintain a Common Access Card (CAC) and pass required background investigations. Working Conditions and Environment This position is on-site in a medical facility. Driving may be required. Physical Requirements The position requires repetitive motion, such as standing, walking, sitting and bending. Light lifting, up to 20 pounds, may be required. Requirements

Candidates must hold an active Registered Nurse license in Vermont and an Associate's or Bachelor's degree in Nursing. Preferred qualifications include two years of nursing or case management experience and familiarity with military healthcare systems.
