The nurse provides medical and medication monitoring and education for members of the ACT/FACT team. They also assist in rehabilitation, substance abuse services, and crisis intervention under clinical supervision.
Candidates must hold a current Louisiana Nursing License and a valid driver's license with reliable transportation. Willingness to work in a community setting with individuals diagnosed with SMIs and Co-Occurring Disorders is required.

The ACT/FACT team nurse acts as a team player and shares in the responsibility of carrying out rehabilitation and support functions and assisting in treatment, substance abuse services, education, support and consultation to families and crisis intervention services under the clinical supervision of staff with professional degrees. The nurse is specifically responsible for medical and medication monitoring and education for those on the ACT/FACT team.
