General Purpose of Job: Provides physical therapy treatment to assigned patients in accordance with the treatment plan of care. Assumes responsibility for creating experienced-based service. Essential Duties: Consistently exemplifies “on-stage” behavior. Is responsible for creating a destination, experience-based service; delivers customized experiences using patient profiles. Provides physical therapy treatment to assigned patients in accordance with the treatment plan of care provided by physical therapist. Instruct patients in safe transfers, activities of daily living, correct ambulation, therapeutic exercises and correct usage of exercise equipment; trains family members and caregivers on various therapeutic procedures and home exercises. Performs physical therapy screenings, including range of motion measurements, assesses patient progress; recommends changes to the plan of care as appropriate. Maintains communication with the Physical Therapist, nurses and physicians to facilitate quality patient care; reports abnormal physiological changes. Documents patient treatments, progress notes and discharge summaries in a timely manner and in accordance with Hospital policies and insurance requirements. Assists in developing, marketing and implementing various Rehab Services programs including, but not limited to, Aquatic Physical Therapy Program and the Encore program. Schedules patient treatments/appointments along with referring patients to other hospital services. Provides guidance and training to appropriate staff regarding home exercise programs. Attends staff meetings and in-service programs as required. Assists in cleaning and maintaining therapy equipment and treatment areas; notifies appropriate personnel of supplies needed to be ordered. Supports and serves as a role model for our mission, vision, values, and customer service initiatives. Adheres to the organization’s policies & procedures, and compliance guidelines.
